Data Request Form

Please use this form to request information from the CEHS Assessment & Accreditation Coordinator.  This form will enhance the services that the office can provide by allowing requests to be prioritized and tracked.  Please complete all of the information on this form.  Please note that there are reports available to run in the Cognos system located at the following link: https://cognos.ship.edu/cognos10/

Log in using your ship email with @ship.edu at the end and your system log in password. 

Click My Home>Click SHIP>Click Dept.  This will display a list of reports available to run.  The most popular report to use for student data would be the STU-000-Student detail report.
