Name Tag Request Form

Shippensburg University supplies name tags to new full-time employees free of charge. Replacements, additional, or name tags for graduate assistants or work-study students cost approximately $7 each. Please provide the cost center for the tags to be charged to.

Allow approximately 4 weeks for processing and delivery.
